Health Care Directive

 A health care directive is a written document that informs loved ones of an individual’s healthcare wishes.  The health care directive allows an individual to name a person or agent to make decisions if they are unable to do so.  A healthcare directive is useful if you become unable to adequately communicate your healthcare wishes. 

Within a health care directive, several sensitive questions similar to those listed below will be taken into consideration:

•  What should happen if you become incompetent or incapacitated and cannot make decisions?

•  If incompetent or incapacitated, what type of health care would you like to receive and for how long?

•  Who should make decisions on your behalf?
